Needs gore. A leg there, a hand here, fingers, guts all over the place. Half a face, brain bits scattered. Blood that doesn't disappear and more blood in general. Sort of Solder of fortune but with 2016 graphics. That would be realistic. What is the age rating?

Well, I do not think Battlefield is going to see mud made of blood any time soon.
Nor I think that you'll get to play in real-like smoke, fog, poison gas environments.
Verdun is much better on that side, but it is not that graphically advanced. So, on one side Battlefield 1 which looks lovely, but does not even aspire to realism.
And on other side Verdun which targets realism, but does not have resources/experience to get in there fully.
One day, you'll run uphill in game and your feet will slip on mud. As you fall down and throw your hands in front of you (while dropping gun), you notice that it is kind of brownish mud. And as you touch down, fingers clawing into that mud, you realize that it is bloody mud. But not today, not with Battlefield.

But I see main issue with realism which prevails in any game today.
Absolutely unrealistic projectile impacts and smoke they create. And same goes for grenades, tank shells, ... All create debris in very unrealistic way and smoke in even more unrealistic way.
I do not mind bad details in objects/textures. But I really mind those out-of-place smoke, fire, explosion effects.
I have seen my share of those but I doubt people making those lovely looking games ever seen even recording of real world explosions, impacts and smoke they create.

Game video shows that technology is there, it is just wrongly implemented.
That's exactly what I was thinking. To be fair, only a few games have had good fire/smoke etc. I seem to remember the fog in Batman Arkham Asylum was really good with physX on and would whip around your cape as you moved through it. Also Bioshock 1 in DX10 mode I remember being astounded that fog didn't clip into everything compared to DX9 mode!

But I see main issue with realism which prevails in any game today.

I absolutely love it. I think it looks amazing! I can hardly wait! I am not very good at Battlefield, its just so much fun and I love the way it looks.
I do wish they would add a bit more gore to the game. Nothing crazy because it would take away from the game style, but adding some would add a bit more intensity and urgency of action in the gameplay.
